TERMS OF SERVICE & INFORMED CONSENT

For services with Gabriela Colletta

Ayurvedic Wellness Counseling is a form of wellness coaching that provides clients with recommendations or suggestions to support the natural self-healing intelligence of the body. The goal of all programs is to create within your body and mind an optimum environment for healing to take place and to maximize the body’s ability to heal itself using the principles of Ayurveda.

Ayurveda is currently considered a form of complementary and alternative medicine in the United States. It is not licensed by any state as a medical practice. All services and treatments provided are complementary or alternative to healthcare services provided by licensed healthcare practitioners. Ayurveda does not replace Western Medical diagnosis or treatment.

Informed Consent

  1. An Ayurvedic consultant is not trained in Western medical diagnosis or treatments; he or she is not a physician, nor a licensed healthcare professional, but rather an Ayurvedic consultant using Ayurveda and holistic wellness principles.

  2. No medical advice, diagnosis or treatment is given by the Ayurvedic consultant.

  3. If I am suffering from a disease or severe symptom that has not been evaluated by a medical doctor or other licensed healthcare professional, I acknowledge that I must be evaluated by a medical doctor. If I choose not to see a medical doctor, by signing this understanding I acknowledge that consultation with a medical doctor has been recommended to me.

  4. An Ayurvedic consultant will not alter my prescriptions without approval from my medical doctor or other licensed primary healthcare provider.

  5. The consultant holds no liability for those supplements not prepared or manufactured by her.

I understand that this consultation and any assessment or information ensuing thereafter is for general educational purposes only and that no claim to medical diagnosis or treatment, or the cure of any medical condition, is inferred or implied. I further understand that this consultation and any recommendations ensuing thereafter should not be construed as a substitute for medical examination diagnosis and treatment, and that I should see a medical doctor or other licensed primary healthcare provider for any physical or mental ailment or complaint that I may have. I understand that an Ayurvedic consultation may include assessment of doshic conditions and related recommendations for dietary, herbal or other lifestyle regimens and that these are designed to be ancillary health aids by which I choose my degree of participation. I understand that Ayurvedic consultants are not qualified or licensed to diagnose, prescribe for or treat any physical or mental illness, and that nothing said in the course of any consultation should be construed as such. I understand that any healing process requires my active participation and is my own personal responsibility.

By purchasing or participating in any of Gabriela Colletta’s Ayurvedic or Health Counseling services, I acknowledge that Gabriela Colletta is not a medical physician. I acknowledge that I should always consult my personal physician before starting any new health program and I will keep my medical physician informed of any and all changes I make in my lifestyle. I acknowledge that any advice given in this consultation is to restore balance to the body, mind, and soul and is not meant to replace any medical treatment I may currently be receiving. I acknowledge that Ayurveda is performed for wellness.

Client Responsibility

The Classes, Courses, Consulting and Products were developed strictly for educational purposes. I understand and agree that I am fully responsible for my use and results from the Products. Gabriela Colletta makes no representations, warranties or guarantees. I understand that results may vary from person to person. I understand that Gabriela Colletta is not responsible for my results or any expenses that I may incur as a result of any of my purchases. Gabriela Colletta assumes no responsibility for errors or omissions that may appear in the Courses, Classes, Consultations or Products.

Ayurveda Payment & Cancellation Policy

Payment Policy: I agree to pay all charges incurred for services rendered at the time of visit or in advance.

Cancellation Policy: Appointments must be canceled or rescheduled at least 24 hours in advance of the scheduled appointment time. Any cancellations made within 24 hours of the appointment time will be charged the full appointment fee. To cancel an appointment please email: gabrielacolletta@gmail.com at least 24 hours before your scheduled session. 

Release of Liability: I acknowledge and release Gabriela Colletta from any and all liability. I further acknowledge that I have read and voluntarily agreed to the above terms of service.
